- Transcript Request Form (pdf) – Since the Office of Records and Registration is currently open through email and phone only, picking up paper transcripts is not an option and therefore, these transcripts must be mailed. Please be aware that the production of paper transcripts will be delayed. When possible, we recommend that students request electronic (PDF) transcripts.
