Immunization Requirements Upload Immunization Records Health Services will only be accepting immunization records that are uploaded to the Patient Portal. Log in using your NetID and school password then verify your birthdate. Select the Forms and Requirements tab on the left side of your home screen and follow the directions to upload your immunizations. If you need to submit a religious exemption for immunizations, that is also located on the patient portal under the Optional Upload Form tab on the left side. 12-01-2025
