To schedule an appointment, you can email at uishealthservice.edu, web book at UIS patient portal, or call 217-206-6676.
All charges will be placed onto student accounts. It is the students’ responsibility to submit any receipts to their insurance provider. The receipts and other necessary forms can be found on the Health Services Patient Portal.
Health Services will only be accepting immunization records that are uploaded to the Patient Portal. Log in using your NetID and school password then verify your birthdate. Select the Forms and Requirements tab on the left side of your home screen and follow the directions to upload your immunizations. If you need to submit a religious exemption for immunizations, that is also located on the patient portal under the Optional Upload Form tab on the left side.

It would require at least one vending machine on public college or university campuses to offer emergency contraceptives as an option.

After a brief consultation and a quick hello on Zoom, Xaria heads back downstairs to consult with dad about more pressing issues like toys.
