With this session, we hope to demystify Leaves of Absence and foster a better understanding among our campus community.Do you have questions related to Leaves of Absences? We want to ensure we have answers! Please use this form to submit questions prior to the session.
This session will be held virtually on August 26, 2025 from 9:00-10:00am.
Reminders
Protection of Minors
Are you hosting an event this Fall? If you have an event on campus that involves minors, please submit the electronic Protection of Minors event form for HR review! The Protection of Minors webpage has detailed information on planning and hosting an event for young visitors. We recommend that you plan on submitting information to Human Resources at least 30 days prior to the event. Please contact uishr@uis.edu if you have any questions about your upcoming event.
Remote Work Agreements
We wanted to give a friendly reminder that Remote Work Agreements are good for up to one (1) year. Remote work can be a viable option for employees in jobs that are suited for such an arrangement. Civil Service and Academic Professional employees working a remote work schedule must keep a current Remote Work Agreement on file with Human Resources. Please use the electronic Remote Work Agreement form to submit your request. You can review the UIS Remote Work Policy for more information, or contact Kenzie Westlake at mwest22@uis.edu if you are unsure if you have a current agreement on file.
Employment Verification
As hiring season is in full swing for the university as well as outside entities, we have also seen a large influx of employment verification requests. We encourage all active employees to utilize the Employment Verification System (EVS). This system allows you access to your own employment verification as well as the option to send your vendor a link to access the EVS and retrieve the information you have allowed. Outside of the EVS, all employment verifications should be directed to HR. Supervisors should not be verifying any factual employment details, including Public Service Loan Forgiveness (PSLF) forms. If your department receives an electronic request for PSLF certification, please forward it to [uishr@uis.edu]uishr@uis.edu. However, supervisors are welcome to provide professional references for employees, if requested.
Workers' Compensation Injury Reporting
Promptly reporting workplace injuries is critical for the health and safety of our employees and for maintaining an efficient Workers’ Compensation process. If an injury occurs on the job, it should be reported as soon as possible, even if it seems minor at the time. Early reporting helps ensure timely medical care, proper documentation, and a smoother claims process. Delays in reporting can lead to complications with treatment, missed benefits, and difficulties in the claims process. If you are injured or witness an incident, notify your supervisor right away and follow the proper reporting procedures. Your quick action protects your well-being and supports a safe workplace for everyone.
Please complete the First Report of Injury form (pdf) as soon as possible following an injury and forward it to our campus Workers’ Compensation liaison, Heather Moore, at hkimb2@uis.edu. For more information on Workers’ Compensation reporting requirements, visit the U of I System website on Reporting Procedures and Responsibilities.
