Where to Get Shots/Immunizations

Here are some locations where you can receive the required immunizations: personal physician office, local health department, VA or pharmacy

Beginning Spring 2019, Health Services will require Yellow Cards (see sample photo) from the World Health Organization as evidence for immunizations administered outside of the United States. Students unable to produce a Yellow Card to verify non-domestic vaccinations may receive the necessary immunizations at UIS Health Services or at any domestic health care provider of their choosing. Students with the UIS Student Insurance plan should determine if these alternate sources are considered Preferred PPO Providers under the Blue Cross Blue Shield PPO network.  This can be determined using the contact information on the student’s insurance card. Please contact Human Resources at studentinsurance@uis.edu with any insurance questions.
