Hazing Policy

Parental Notification Policy – The decision to give parental notification will be based on factors such as the student’s prior judicial history, the severity of the alleged incident and other relevant circumstances.

Student Conduct Code – The Code covers disciplinary action initiated or taken against a student.  Disciplinary action may be initiated for proscribed conduct occurring on property owned or controlled by the Campus or conduct in the course of participating in a Campus-sponsored program or project.

Student Grievance Code – The Code covers a grievance brought by a student against another member of the Campus community.

Students – Designate a “Primary Contact if Missing” – The Clery Act contains certain requirements related to students who have been determined to be missing more than 24 hours. One of these requirements included providing all students with an opportunity to name an emergency contact that is specifically related to and used when a student has been determined to be missing. This is separate from your normal emergency contact. I encourage you to identify a contact for this purpose.All active UIS Students can designate a “Primary Contact if Missing “ individual by following the instructions noted below. Designating a “Primary Contact if Missing” via Student Self-Service:

  1. Log into Student Self-Service,
  2. Click on the “Personal Information” link,
  3. Click on the “Emergency Contacts” link,
  4. Click on the “New Contact” link,
  5. Select the “Primary Contact if Missing” relationship designation from the pull-down list, and
  6. Enter all requested information (e.g., name, address) and click “Submit Changes.”
