Forms for Financial Aid
To Send Documentation to Our Office
Upload via myUIconnect, under Financial Aid Home page in the Financial Aid card.
- Log in to the UIS Self-Service system
- Click on "Financial Aid Home Page" link in the Financial Aid card
- Make sure you are looking at the correct aid year (viewed at the top of the page)
- Below the Requirement you are submitting documentation for, click the Choose file button
- Choose the file you are needing to upload and click open (.pdf, .jpg and .png file types)
- Once you see the file pending next to the Choose file button --- Click Submit
- If the document has been uploaded successfully, you will see a “Document Upload Successful” status appear in the yellow box just to the right of the requirement’s link
