Athletic Training Professional Practice
Post-Acceptance Requirements
Once accepted to the Master of Athletic Training Program students will be required to submit the following documents:
- Technical Standards Document
- Verification of completion of HIPPA Training
- Verification of completion of FERPA Training
- Signed Confidentiality Form
- Proof of the following immunizations:
- Varicella immunity: established by date of vaccination or quantitative antibody titer
- Rubella, Rubeola, & Mumps immunity: established by two reported dates of vaccination or documented quantitative antibody titer
- Hepatitis B immunization: established by three reported dates of immunization or by documented testing of quantitative antibody titer
- Tetanus diphtheria: every 10 years, established by reported date of immunization
- Annual PPD (purified protein derivative) skin test or the IGRA (interferon-gamma release assays) blood test.
- Both methods are used to diagnose silent (latent) tuberculosis (TB) infection.
- If the test is considered positive a Chest radiograph is required, every three years.
- Annual influenza vaccine: established by reported immunization date
- Completion of a Criminal Record Background Check and/or verification of liability Insurance may be required depending on the practicum site assigned.
