Campus-Wide Survey Requests

The University of Illinois Springfield has adopted a campus-wide survey policy in order to protect our community members from being over-surveyed.

If you would like to conduct a survey of members of our campus community (students, faculty, staff, alumni), please complete the form and submit to ora@uis.edu. The Institutional Data Steering Committee will review the application and make recommendations to the Associate Vice Chancellor for Research and Institutional Effectiveness, who will coordinate with various campus units in terms of scheduling surveys and sharing results.

You may also need approval by the Institutional Review Board.  Please review the material on their website and complete the appropriate form, if needed.
