The Illinois Community Action Association has contracted with the Survey Research Office of the University of Illinois to conduct an evaluation survey of its two newsletters, Homeless Headlines (the monthly newsletter) and Homeless Hotline (grant and workshop announcements).

Your opinions about the newsletters, including your opinions about what you find useful in them, are important and will help ICAA address the information needs of service providers and advocates throughout Illinois.

Hard copy or website version.
You can complete the hard-copy questionnaire that was sent to you through the mail. Or, you can complete the website version by clicking on “ICAA Newsletter Survey – Website Version” below.

Project identification number and confidentiality.
You will first be asked for your project identification number. This is the number next to the date on the letter you received (and also the number on the front page of the questionnaire you received). This is for record-keeping purposes only. No one at ICAA will know the responses you give.
