In addition to the University application, students must submit a separate application to the Master of Public Health program. The application forms are provided here in Portable Document Format (pdf). You will need the Adobe Reader to view these files. The Reader can be downloaded free from the Adobe website.

To be considered for admission to the Master of Public Health program, PLEASE SUBMIT:

  • Graduate Records Examination [GRE] scores
    Other aptitude tests - Medical College Admission Test (MCAT), Dental Aptitude Test (DAT), Law School Admission Test (LSAT), or the Graduate Management Admission Test (GMAT)- can be submitted. Applicants with post-baccalaureate degrees are exempt from this criteria.
  • Letters of recommendation (.pdf)
    Applicants are expected to provide 3 letters of recommendation from employers, peers or educators.
  • Goal statement
    Applicants must submit a separate statement of their professional goals, identifying past academic work and/or employment related to these goals.

Note on Confidentiality: Applicants for admission do not have access to their application records. Under the provisions of the Family Educational Rights and Privacy Act of 1974 only registered students and alumni have access to their educational records. This report will be used solely for the purpose of determining whether or not the applicant should be admitted to the public health department. After the admissions committee has made its decisions, this reference report and all others written on the applicant's behalf will be removed from the file and will not be a part of the student's educational record if he or she matriculates to UIS.
