parking decal application forms and then print the form
to submit to the UIS Bursar's Office. Select the
appropriate form below. UIS 2007-2008 Student Parking Decal Application Form UIS 2007-2008 Faculty/Staff Parking Decal Application Form Instructions: Once the form is downloaded into Adobe Acrobat Reader, click into the Last Name field and begin completing the form. You may use the Tab key or Shift+Tab keys to move forward or backwards to each field. Once you complete the form, print the form on white paper. Be sure to sign and date it at the bottom. Take the form to the UIS Bursar's Office room PAC 184 or mail it along with your check or money order to: |
