6.0 or better to fill out the Petition
for Appeal of a Parking Violation Ticket Form online and then print the form
to submit to the UIS Police Department.
Once the form is downloaded into Adobe Acrobat Reader, click into the Name field and begin completing the form. You may use the tab key to advance to each field. Be sure to complete all information on the form. Once you complete the form, print the form and sign it. Mail the appeals form to the address on the form or bring to the UIS Police Department. |
