Medical Care: Health Information and Health Insurance
For your protection and easy transition to UIS, please complete and have signed by your health care provider all required immunization and health records.
Major medical expenses are very costly and health insurance coverage is mandatory at UIS. However, student insurance will not cover the cost of eye examinations, glasses or dental work. It is therefore advisable that you obtain this health care at home, before your departure. Consider bringing an extra pair of eye glasses or contact lenses, and the original optometric prescription in case of damage or loss.
F-1 Visa students: International students are eligible for UIS student group health insurance and an insurance fee will be assessed at registration. Family coverage, as well as maternity benefit coverage, may be purchased at additional cost.
J-1 Visa students: The U.S. information Agency (USIA) requires that all persons in J status have adequate health insurance while in United States. Mandatory minimum coverage shall provide: medical benefits of at least $50,000 per accident or illness; repatriation of remains in the amount of $7,500; expenses associated with the medical evacuation of the exchange visitor to his/her home country in the amount of $10,000; and a deductible not to exceed $500 per accident or illness.
UIS student group health insurance does not meet all USIA requirements and J Visa students are required to purchase individual health coverage policies; information and application forms are available in the Office of International Programs.
