- Student Insurance Information - All students enrolling for one (1) or more hours per semester are automatically assessed* the student insurance fee and enrolled in the student insurance program unless a Student Health Insurance Petition for Exemption (pdf), accompanied by proof of insurance, is submitted to Human Resources by the September 10, 2009 deadline.

If you submitted a Petition for Exemption form, along with proof of insurance, during a previous semester there is no need to resubmit for Spring 2009.
* ONLINE students are not eligible for student insurance.
