Workers compensation

caution signUniversity employees with injuries arising out of and in the course and scope of their employment duties are eligible for Workers’ Compensation benefits. Workers’ Compensation insurance includes four types of benefits – disability (loss of income) benefits, medical care benefits, survivor (death) benefits, and rehabilitation benefits. In some instances there may also be some type of permanent disability payment.

Reporting Work-Related Incidents/Accidents:

  • Contact Your Supervisor

An employee who is injured on the job must inform the employer promptly. Any delay in notifying the employer can delay payment of benefits. A delay of more than 45 days may result in the loss of all benefits. Notice to a fellow worker who is not a supervisor or otherwise a part of management is not considered notice to the employer.

  • Complete First Report of Injury/Illness

An employee must thoroughly complete and sign the First Report of Injury/Illness form within 24 hours and forward to their supervisor. The supervisor must also thoroughly complete the supervisor’s portion and promptly forward the completed form to Human Resources, HRB30, or fax to 206-7145.

Also, if medical treatment was necessary, the Authorization for Medical and Health Care Information form must be completed and forwarded to Human Resources, HRB30.
