UIS Immunization Policy

Immunization Compliance

Immunization forms can be obtained from Campus Health Services or downloaded from the UIS website at 44457RequiredImmunizationForm.pdf (pdf) . The form must be complete the top box filled out and signed, and on file in Campus Health Service by 5 p.m of the 10th official day of the fall and spring semesters or the 5th official day of the summer term.

If a record is not complete and on file in the CHS by that date, students will be assessed a non-refundable $25 non-compliance fee. If a student is not in compliance by their second semester at the UIS, the college immunization law does not allow them to register for the 2nd semester.

Immunizations are in compliance when the following occurs:

  1. Immunizations are turned in on or before the deadline.
  2. Immunizations are complete and in order;( Read Illinois State Law.)
  3. Immunizations are verified by the signature of an R.N./M.D.; ( Read Illinois State Law.)
  4. Campus immunization form is completed and signed by student; or if under age of 18yrs by parent or legal guardian.
    ( Please read UIS Service regulations & the Immunization Form to be sure you have complied with all of the requirements. )

UIS Immunization Policy

All students must provide proof of immunity to the following diseases by 10th day of classes in Spring and Fall and 5th day of classes in Summer:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Tetanus/Diphtheria
  • Students who are not US citizens must have a TB skin test done in the US at a Public Health Department or University Clinic or the UIS CHS no earlier than 60days before registration.

RUBEOLA(Measles):

Two doses of live vaccine or two doses of MMR separated by at least one month are required. The first dose must be given after January 1, 1968 and on or after your first birthday. MMR is preferred and must be given as of May 1, 1971, and on or after your first birthday!
  • Blood test/titer may be done to prove immunity. A copy of the report must accompany the immunization form.
  • Proof of having had the disease : Must include the day-month-year disease was diagnosed & the diagnosing physician's signature and address.

RUBELLA: German Measles

  • One dose of live vaccine must be given after January 1, 1968 and on or after your first birthday.
  • Blood titer may be done to prove immunity. A copy of the report must accompany the immunization form.
  • Having had the disease is NOT an acceptable proof of immunization.

MUMPS

Two dose of live vaccine must be given after January 1, 1968 and on or after your first birthday.

Blood titer may be done to prove immunity. A copy of the report must be sent to our office. Acceptable Titers are:

  • ELISA

Proof of having had the disease : Must include the day-month-year disease was diagnosed & the diagnosing physicians signature and address.

MMR:

Measles, mumps, rubella; will satisfy proof of rubeola, rubella, and mumps innoculations if the guidelines below are satisfied:

Two doses of MMR:

Shots must be separated by more than one month.
First dose must have been given as of May 1, 1971, and on or after your first birthday!

Tetanus/Diphtheria:

Three doses of Tetanus/Diphtheria are required, with the last dose given within the last 10 years.

Students who are not U.S Citizens

Students of all ages are required to provide documentation of three doses of tetanus diphtheria with the last dose given in the past ten years. They are required to maintain a current Tetanus Diphtheria while they attend UIS.

Tuberculosis (TB)

Students who are not U.S Citizens

Students who are not U.S. citizens must have a tuberculin skin test done at the Campus Health Service on arrival and prior to registering. If the test is positive, they will be required to have a chest x-ray when they arrive on campus.

International students who have had a positive tuberculin skin test in the past are required to bring a statement from the health care provider who performed the test, giving the date of the test and the size of the induration in millimeters. They will be required to have a chest x-ray when they arrive on campus. All medical records must be accompanied by a certified English translation.
