TIME SHEET

All GRADUATE ASSISTANTS are required to keep record of the actual hours worked on a time sheet. The supervisor must review, sign, and maintain a copy of the time sheet on a monthly basis.

All PRE-PROFESSIONAL ASSISTANTS, RESEARCH ASSISTANTS, and TEACHING ASSISTANTS are considered "salaried exempt" appointments. A time sheet is not required, but is advisable for both the assistant's and department's records.

 

ASSISTANTSHIP CALENDAR of EVENTS FOR 2009-2010 ACADEMIC YEAR

 

PETITION/WAIVER FORMS for ASSISTANTS

  1. Waiver for Maximum Term of Appointment (Filed to request permission to waive the Assistantship Policy restiction limiting the number of semesters of appointment to a maximum of 4. A Graduation Contract should accompany this petition as supporting documentation.)
  2. Waiver for Maximum Course Load (Filed to request permission to waive the Assistantship Policy limiting the total number of credit hours an assistant can be registered for during the fall or spring semester to no more than 12 hours.)
  3. Waiver for Minimum Course Load (Filed to request permission to waive the Assistantship Policy limiting the total number of credit hours an assistant can be registered for during the fall or spring semester to no fewer than 8 hours.)
  4. Request for Summer Tuition Waiver (Please note that the Graduate Assistantship Policy Manual states that a student who has served as an assistant during a regular semester preceding a summer session of the same academic year is eligible for a tuition waiver of up to 6 hours during that summer term, as long as the assistant does not exceed the maximum of 30 credit hours of tuition waived during the academic year.)
  5. Assistant's Request for Reappointment (Filed during the spring semester of an assistantship appointment to notify the GA Office of your desire to either continue in your current placement during your second year, or to inform the office of why you do not wish to be reappointed. Should be filed by February 19, 2010.)
  6. Waiver for Incomplete Coursework (Filed to request permission to waive the Assistantship Policy restriction which prohibits assistants from continuing their appointments when they have received incomplete grades - except for thesis or closure project courses.)
  7. Required Documentation of Graduate Hourly Work (Filed during the fall and/or spring semester when the assistant wishes to request an exception to the 20 hour maximum work policy to work in limited occasional employment at UIS in a "grad hourly" position for no more than 5 hours per week for a specified, limited time period . Please note that international students hired as graduate assistants or interns are not eligible to file this petition as they may not work more than 20 hours per week during periods when classes are in session due to visa restrictions.   8. Request for Concurrent Registration Tuition Waiver (Filed when the assistant wishes to request to concurrently enroll at either UIUC or UIC during the fall or spring semester.)
  9. Documentation of Academic Plan for Next Semester (Filed when one or more of the following situations occurs: 1) the assistant's pattern of course completion fails to meet the minimum course load requirements; 2) the assistant's cumulative grade point average (GPA) falls below 3.0; and/or 3) the assistant is placed on academic probation or suspension. This form is completed in consultation with the assistant's academic advisor.)
  10. Notice of SUMMER Graduate Hourly Work (Filed when an assistant who has been serving during the fall and/or spring semester wishes to work during the summer on campus in a graduate hourly appointment. The assistant may work up to 40 hours per week during the summer period.)

 

ENDORSEMENT FORM for SUPERVISORS

  1. Assistantship Reappointment Endorsement (Filed during the spring semester to notify the GA Office of the hiring department's desire to continue the assistantship appointment of the current assistant for a second year, or to inform the GA Office of why you wish not to reappoint the current assistant. Should be filed by March 1, 2010.)

 

EVALUATION FORMS

  1. Mid-Year Evaluation Form (Due to GAO by December 11, 2009.)
  2. End-of-Year Evaluation Form (For Fall 2009 graduates, due to GAO by December 11, 2009. For all other assistants, this should be submitted at the end of the 2009-10 contract period, and is due to the GAO by May 3, 2010.)
  3. Exit Survey (Filed by assistants during the final semester of the assistantship.)
