Faculty Referral Forms
The CTL has created Faculty Referral Forms so that faculty can recommend individual tutoring for students who might be struggling in their classes. Faculty can save the Word Document to your computer and edit the discipline-specific topics or download the PDF if the topics don't need editing. Faculty can complete these forms and give them to their students as a means to follow through with recommended tutoring appointments. After the student has met with their tutor, the student can then return the completed form to his/her professor. Please contact the CTL with any questions about this process.
