Request for Certification
Candidates completing approved professional education programs at University of Illinois Springfield should complete the appropriate form online, print it out, sign it, and return TWO originals to their program office. Faxed copies and electronic submissions will NOT be accepted.
Candidates for subsequent certification shoould follow the directions on the form.
