Instructors ordering course packets to sell in the Bookstore that include copyrighted material need to furnish the following at the time of submission: 1) a completed Campus Services Request Form and 2) a completed Copyright Permission Request Form. These items should be forwarded to Campus Services, BSB 139.

Copyright Permission Request Form
The following information is necessary to request copyright clearance:
- Title of work (book or journal)
- Title of the chapter or article
- Author's name
- Publisher
- Date of publication
- Exact page numbers
- Any other information, such as copies of title pages, etc.
Please allow 6-8 weeks for copyright clearances to be processed.
