UIS Sabbatical Application Guidelines
Step 1: Complete the UI Application for Sabbatical Leave form. Note that Page 3 of the form references both a concise statement of plans/purpose of leave (limited to 40 words) AND an expanded statement of plans (limited to no more than 1000 words) using the sections provided on Page 5 as a guide for information to be included.
Step 2: Make a copy of your Curriculum Vita, to be submitted along with the completed UI Sabbatical Application form. Although the VPAA’s guidelines for sabbatical leaves of absence specifically direct applicant not to include curriculum vitae, please note that UIS requires that a current vita be submitted along with the completed UI Application for Sabbatical Leave form.
Step 3: Submit the completed UI Sabbatical Application form and the copy of your Curriculum Vita to your Dean by September 20, 2013.
Step 4: Departmental Review and Recommendation. The Dean will refer the sabbatical application materials back to the respective academic department chairs for departmental review and completion of the Statement of Unit Executive Officer on Page 4 of the UI Application for Sabbatical Leave form. If the recommendation is affirmative, Unit Executive Officer must indicate (a) what provisions will be made to ensure workload coverage (including teaching, thesis supervision, academic advising, and any other departmental assignments) and (b) any additional university expense related to the sabbatical leave, including hiring of adjuncts to ensure delivery of the curriculum if the leave is granted, (c) rank the application, and (d) sign the request. Academic department chairs return the application materials to the Dean by October 15, 2013.
