The Campus Police seek to provide a safe and secure campus environment by developing, coordinating, and enforcing policies and procedures related to safety, security, service, and law enforcement (including parking and traffic regulations). The department provides round-the-clock assistance to students, faculty, staff, and visitors.

Any serious injury or illness should be reported to the Campus Police and the Campus Health Service immediately. Any dangerous, suspicious, or criminal activity should be reported to the Campus Police immediately.
UIS POLICY & PROCEDURES PROHIBITING VIOLENCE ON CAMPUS
POLICY STATEMENT
The University of Illinois at Springfield is committed to a safe and secure environment for its students, employees, guests, and visitors. Threats, threatening behavior, or acts of violence against students, employees, visitors, guests, or other individuals by anyone on UIS property or related to university activities will not be tolerated. Violations of this policy will lead to disciplinary action which may include reprimand, suspension, dismissal, arrest, and prosecution.
Threats, threatening behavior or acts of violence are defined as any act or conduct that a reasonable person would perceive as threatening or violent. Examples include but are not limited to behavior that:
· Is physically assaultive
· Consists of a communicated or perceived threat to harm an individual or to endanger the safety of an individual
· Harms or destroys property
· Consists of a communicated or perceived threat to harm or destroy property
· Involves carrying or displaying weapons (except for duly authorized peace officers).
All employees who have been subjected to or witnessed threats or violence must report such incidents to appropriate campus personnel.
It is a violation of this policy to retaliate against a person for reporting threats, threatening behavior, or acts of violence or for assisting, participating, or cooperating in an investigation of same. When retaliation is charged, the complaining party may use the grievance mechanism applicable to his/her employment status. Retaliation is a basis for disciplinary action or sanctions.
PROCEDURES
REPORTING A CRISIS INCIDENT
An Employee subject to or witnessing threats, threatening behavior or an act of violence that poses imminent danger to the safety of an individual shall immediately report the incident to Campus Police for assistance (206-7777).
Employees should not jeopardize their safety, or the safety of others, by attempting the removal of a person who is engaging in violent acts without the assistance of law enforcement personnel.
REPORTING OTHER INCIDENTS
Any employee who is subject to or witnesses threats, threatening behavior or an act of violence shall report the incident to his/her supervisor. This information should be reported regardless of the relationship between the individual who initiated the threatening or violent behavior and the person or persons who were the focus of that behavior.
If the supervisor believes the reported behavior poses an imminent danger to the safety of an individual, he/she shall call Campus Police (206-7777) for assistance or advice.
MANDATORY FILING OF INCIDENT REPORT FORM
Whenever a report is made, the employee, with the assistance of the supervisor if desired, shall fill out a Threat/Violent Incident Report form and file it with Campus Police within 48 hours of the incident.
Campus police will forward a copy of
the Incident Report to the supervisor of the alleged aggressor for review and
possible disciplinary action according to routine procedures. The supervisor shall notify his/her Division
Head of the incident. If the alleged
aggressor is a student the copy of Incident Report should be sent to the Vice
Chancellor for Student Affairs.
NOTICE OF ORDERS OF PROTECTION
All employees who have been granted an order of protection or restraining order which lists any UIS location as a protected area must provide a copy of the order to his/her supervisor. Any employee who has an order of protection or restraining order issued against him/her, which lists either any UIS location as a protected area or any UIS student or employee as the protected individual, must provide a copy of the order to his/her supervisor.
Although the University understands the sensitivity of the information requested and recognizes and respects the privacy of the reporting employees(s), supervisors shall obtain, when possible, a photograph and description of the potential perpetrator, license number of his/her vehicle, along with the type and color of the vehicle.
Supervisors will send the Order of Protection to the Chief of Campus
Police. Supervisors may also provide this information to other employees who
the supervisor determines has the need to be aware of the potential threat.
Every effort shall be made to maintain confidentiality.
VICTIM ASSISTANCE/REFERRAL
Brochures providing information on victim assistance are available at Brookens Library, Human Resources, Counseling Center, Women’s Center, Health Services, Vice Chancellor for Student Affairs, Housing Commons, and Campus Police Department.
For reported incidents, Campus Police will provide information on referrals for victim assistance. If Campus Police have not been contacted with respect to an incident, the supervisor should make the information available.
POST-INCIDENT MANAGEMENT
After a reported incident of violence, Campus Police, the Supervisor, or the Vice Chancellor for Student Affairs, as appropriate, shall ensure that other individuals who may have been affected by the incident, such as employees, students and witnesses, receive information about the availability of counseling services or referrals from the UIS Counseling Center.
PROSECUTION/DISCIPLINE SUPPORT
To discourage further incidents from occurring and to show support for victims of violence, supervisors and faculty should support discipline or prosecution efforts by making reasonable accommodations to schedules so employees or students may cooperate with investigations or attend hearings or court appearances.

POSSESSION OF WEAPONS POLICY
Illinois State Statute provides as follows:

Unauthorized possession or storage of weapons
a. Whoever possesses or stores any weapon enumerated in Section 33A-1 in any building or on land supported in whole or in part with public funds or in any building on such land without prior written permission from the chief security officer for such land or building commits a Class A misdemeanor. (Class A misdemeanors are punishable by 6 to 12 months in jail or up to two years probation, and a fine up to $2,500.00)
b. The chief security officer must grant any reasonable request for permission under paragraph (a).

Definitions
c. "Armed with a dangerous weapon". A person is considered armed with a dangerous weapon for the purposes of this Article, when he or she carries on or about his or her person or is otherwise armed with a Category I, Category II, or Category III weapon.
d. A Category I weapon is a handgun, sawed-off shotgun, sawed-off rifle, any other firearm small enough to be concealed upon the person, semiautomatic firearm, or machine gun. A Category II weapon is any other rifle, shotgun, spring gun, other firearm, stun gun or taser as defined in paragraph (a) of Section 24-1 of this Code, knife with a blade of at least 3 inches in length, dagger, dirk, switchblade knife, stiletto, axe, hatchet, or other deadly or dangerous weapon or instrument of like character. As used in this subsection (b) “semiautomatic firearm" means a repeating firearm that utilizes a portion of the energy of a firing cartridge to extract the fired cartridge case and chamber the next round and that requires a separate pull of the trigger to fire each cartridge.
e. A Category III weapon is a bludgeon, black jack, slungshot, sand-bag, sand-club, metal knuckles, billy or other dangerous weapon of like character.
For the purpose of
implementing the provisions of this Statute for the University of Illinois
campuses and all properties administered under the auspices of the University of
Illinois the following regulations and procedures are hereby established.
The University of
Illinois hereby designates the Chiefs of Police of the University of Illinois
at Urbana-Champaign (UIUC), University of Illinois at Chicago (UIC) and
University of Illinois at Springfield (UIS) as the Chief Security Officer,
pursuant to 720 TLCS 5/21-6, to receive, review and issue written
permissions to possess or store weapons on University property, in accordance
with the provisions contained herein.
Persons seeking
permission to possess or store any weapon in or on University buildings or land
shall direct such request to the Chief of Police of the University campus on
which such building or land is located. If the University building or land is
not located within one of the three specified campuses the requestor shall
direct such request to the Chief of Police of the campus that is responsible
for the management of the University building or land for which the request is
being made.
A request for permission to
possess or store a weapon must be submitted in writing to the appropriate Chief
of Police. Said request shall contain the following:
1.
The name, address,
and phone number of the requestor;
2.
A full and complete
description of the weapon(s) involved, to include make, model, serial number
and all other relevant descriptors;
3.
The relationship
that the requestor has to the University building or land involved (student,
faculty, employee, visitor, guest, etc.);
4.
A detailed
description of the purpose of the request;
5.
Copies of all
relevant weapon documentation (e.g. licenses, registrations, proof of
ownership, etc.)
6.
Specific
dates/period of time requested; and
7.
Complete description
of the proposed mode of transportation to and from University building or land
and secured storage of said weapon(s).
Any changes in the above information or permanent removal of the weapon(s) from campus buildings or land needs to be communicated in writing to the appropriate Chief of Police. There are separate requirements for purchasing, recording, and disposing of weapons acquired with University funds. Please refer to sections 7.3 (Cash Purchases), 12.2 (Capitalization and Valuation of Property and Equipment), and 12.4 (Acquisition, Transfer, and Disposal of Equipment) of the Business and Financial Policies and Procedures. The Chief of Police
receiving a request for permission to possess or store a weapon shall:
1.
Review each request
for completeness and verify information contained therein;
2.
Ensure that the
proposed possession/storage/ mode of transportation would not entail a
violation of any Federal, State or local laws or ordinances;
3.
Ensure that the
granting of such request would be in compliance with existing University
regulations;
4.
Determine the
reasonableness of the request.
5.
A denied request
shall be in writing, directed to the requestor, outlining the reason(s) for
such denial.
6.
An approved request
shall be in writing, directed to the requestor, specifying that approval is
contingent upon the requestor delivering the weapon(s) and specified original
relevant documentation (licenses, registration, proof of ownership, etc.) for
inspection to the Chief of Police, or his designee.
7.
Upon successful
completion of 6 (above) the Chief of Police will provide the requestor with
written instructions as to the
possession/storage of the weapon(s) in question to include duration, location,
restrictions, use and limitations.
8.
The Chief of Police
shall ensure that appropriate storage facilities are available for the safe
storage of any requested weapon(s).
Lack of appropriate storage facilities at the requested location shall
constitute sufficient reason for denial of such request.
9.
The Chief of Police
can revoke approval for non-compliance of the requestor, change in
circumstances, change in law or University regulations, or any other valid
reason.
The following
circumstances shall constitute written permission for the possession/storage of
weapons:
1.
Registration in any
course of study where weapons are used in connection with that course for such
students to use any University owned or controlled weapons when such weapon is
used in connection with that course of study;
2.
Employees or agents
of the University who are required by the University to possess or control
weapons are authorized to possess or store such weapons in the performance of
their duties;
3.
Members of the Armed
Services or Reserve Forces of the United States of America or the Illinois
National Guard, who are authorized to possess weapons under the laws of the
State of Illinois, while in the performance of their official duties;
4.
Sworn local, state
or federal peace officers, who are authorized to possess weapons under the laws
of the State of Illinois, while in the performance of their official duties as
designated by their respective law enforcement agency;
5.
Wardens,
superintendents and keepers of prisons, penitentiaries, jails and other
institutions for the detention of persons accused or convicted of an offense,
who are authorized to possess weapons under the laws of the State of Illinois,
while in the performance of their official duty;
6.
Guards of armored
car companies, who are authorized to possess weapons under the laws of the
State of Illinois, while actually engaged in the performance of their duties as
contracted by employees or agents of the University.
7.
Off-duty sworn
local, state or federal peace officers, who are authorized to carry weapons off-duty
both under the laws of the State of Illinois and by authority of their
respective agency, while engaged in rendering services for the university. This
provision will only be applicable if the University unit/agency engaging said
services have first received approval from the Chief of Police. Said
unit/agency will provide such information relating to the proposed activities
of such officers as requested by the Chief of Police.

Health Service
The Campus Health Service is available to the entire campus community. Its primary purpose is to serve both full- and part-time students whether or not they have student health insurance. Students receive priority during busy times. There are no mandatory fees for the use of the Campus Health Service; however, minimal user fees are charged for some services and patients are informed about these fees before the service is rendered. Nurse or physician visits and many other services are provided without charge. Health resources such as literature, web sites, videos, books, and a self-care cold center are available in the office. Information pamphlets are also supplied on racks at various locations on campus.
A registered nurse is available during regular hours except when the campus is officially closed. Physicians see patients by appointment only, three to six hours a week. For UIS student insurance purposes, referrals are made to other physicians and medical services only when the campus health service cannot provide treatment.
ALL CARE IS COMPLETELY CONFIDENTIAL. Access to medical records is provided by written consent of the patient and as required by law.

· Treatment for minor injuries and acute illnesses
· Physical examinations for all UIS athletes and for students who are required to have them for work, school, etc.
· Birth control, pelvic examinations, and pap smears
· Referral for problem pregnancies
· Required immunizations
· Individualized weight control programs

· Hepatitis B vaccine at a low cost
· Required and recommended immunizations for foreign travel. There is a fee for travel consultations and immunizations provided to employees; students pay only for travel vaccines.
Note: Laboratory tests are done at local hospitals
and are paid for by the patient or the patient’s insurance plan.
Students and employees who need care when the Campus Health Service is not open should go to either an emergency room or an urgent care center. Care should be taken to select a source according to health insurance coverage requirements. A list of the local urgent care centers is available from the Campus Health Service office.

Insurance
The University of Illinois is self-insured for liability claims. The self-insured program provides protection for employees for claims arising from the performance of their University-related duties. Claims arising from personal activities or other employment are not covered. Questions or information should be directed to Administrative Services.
Liability claims against the campus or its employees are handled in the Office of Administrative Services. Investigations, which may result in liability claims, include automobile liability, general liability, and workers’ compensation. The Office of Claims Management for the University of Illinois is consulted by the staff of Administrative Services as needed.
Any employee who is sued or who receives a summons or other legal complaint pertaining to their University-related duties must deliver the complaint to Campus Legal Counsel as soon as possible, but no later than 15 days after receipt. The furnishing of all legal services, including legal defense, is the responsibility of the University through its legal counsel office, which may appoint outside legal counsel. The University is technically the insured party, with staff members covered as employees engaged in University-related duties. Further information may be obtained from Campus Legal Counsel.

Effective with the Fall 2004 semester, all students who are physically attending classes on campus (Springfield and Peoria) will be automatically assessed a non-refundable fee for student health insurance. Students with alternative health coverage, may submit a ONE-TIME ONLY Student Insurance Exemption, which will prevent future assessments for Student Insurance provided by the University. If you have submitted an exemption form during either the Fall 2003 or Spring 2004 semester, your exemption will remain on the system and no further documentation is required at this time. However, students who have submitted exemption forms prior to the Fall 2003 semester will need to submit a new exemption form in order for us to populate Banner with the exempt status. Note: Distance learners will remain ineligible for participation in the student insurance program because they are not physically on campus. In addition, the following procedures have been implemented effective Fall 2004:
Students with private insurance or insurance provided by an employer and requesting to waive the automatically assessed student insurance fee will be required to provide hard-copy proof of equivalent coverage to the Office of Human Resources in order to have the assessment removed from the student's account.
Hard-copy proof of equivalent coverage (a current insurance card, letter of verification from the student's private insurance carrier on company letterhead, or a certificate of coverage from the private insurer) will only be accepted as of the established deadline published in the online UIS Course Schedule each semester. The deadline to submit exemptions for the Fall 2004 semester is September 23, 2004. Proof of other coverage may be submitted to the Office of Human Resources either in person; by fax; email with attached proof of other coverage; or postmarked US mail. The Office of Human Resources will keep a copy of such proof and provide the student with a disclaimer showing satisfactory completion of the student insurance exemption process.
A Student Insurance Exemption will continue in effect until such time as the student requests reinstatement to the University's Student Accident and Illness Plan. The Office of Human Resources reserves the right to require "exempt" students to respond to periodic requests to confirm that he/she continues to be covered by another health plan. A student may become eligible for reinstatement to the University's plan within 30 days after involuntary loss of other insurance coverage. Proof of cancellation is required at the time of application. The insurance premium will not be pro-rated for a partial semester of coverage. For reinstatement of insurance coverage, students must pay the entire semester premium.
Only the Office of Human Resources will handle, accept, and issue student insurance exemptions. No other campus office or department will be able to process the removal of a Student Insurance fee. Students should contact the Office of Human Resources at (217) 206-7144 for instructions to complete the Student Insurance Exemption process.
The automatic assessment for student insurance will occur during the Fall, Spring, and Summer semesters for students who register with a minimum of one credit hour. Currently, automatic assessments only occur during Fall and Spring semesters.
When a student enrolls after the tenth day of class, Banner will automatically
assess the Student Insurance fee. Students registering late will have five
business days to complete the exemption process in order to have the assessment
removed from the student's account. Students who need to enroll dependents
or continue student insurance coverage (taking a semester off from classes, i.e.
summer session) are required to complete a paper enrollment form which may be
obtained either in the Office of Human Resources, HRB-30, or the Bursar's
Office, PAC 184.

Inclement Weather, Liberal Leave & Essential Services Policies (Revised November 28, 2007 and approved by Chancellor’s Cabinet)
The UIS campus does not close due to inclement weather except under the most extraordinary conditions.
A decision to cancel classes due to inclement weather is made separately from the consideration to close the campus. So it is possible that classes would be canceled but the campus would remain open for other business.
When classes are canceled, the normal expectation is that all offices and other services of the university will have regular operating hours.
It is also generally assumed that if classes on campus are canceled, online classes would not be interrupted.
If severe weather threatens, the announcement to cancel classes or close the campus is posted officially on the UIS Web site (www.uis.edu) and broadcast on WUIS/WIPA radio (91.9 and 89.3 FM). Official information will also be available at the main UIS switchboard – 217.206-6600, and delivered to faculty, staff and students through official UIS email. Other media will be provided information, but those media are not official sources.
Ordinarily, when threatening weather begins during the day, the decision to cancel evening classes is made by 3 p.m., and when the inclement weather begins during the night, the decision to cancel classes for the day is made by 6 a.m. Decisions about class cancellations may be made at any time if the situation makes it necessary.
The Liberal Leave Policy will be implemented when conditions warrant.
When the campus is declared closed, essential services will still be provided, as spelled out in the Essential Services Policy.
Because UIS students commute from various locations, weather conditions may vary between their home area and the Springfield campus and/or the Peoria Center. If classes are not canceled and a student believes that travel will be dangerous, the student should make the appropriate personal decision and notify his/her instructor of a planned absence from class. Instructors are advised to abide by each student’s judgment in such cases.
Cancellation of classes held in UIS Peoria Center is handled separately.
Liberal Leave Policy
The Liberal Leave Policy may be invoked due to inclement weather, civil disorder or other emergency circumstances.
This policy recognizes that UIS employees may believe that in such situations, it would be dangerous for them to drive or ride to work, or it would be dangerous for them to drive or ride home unless they left before the end of the workday. In such cases, employees should notify their supervisors and request time off for all or part of the day, using either vacation time (or, if no vacation time is available, unpaid leave). Supervisors are advised to abide by each employee’s judgment when the Liberal Leave Policy is in effect.
The chancellor or his designee makes the decision to implement the Liberal Leave Policy. It becomes effective by being announced officially on the UIS web site, via campus email, WUIS radio and/or on the main switchboard number, 217.206.6600.
Supervisors of “essential services” as defined by the Essential Services Policy are responsible for determining which employees must report to work even when the Liberal Leave Policy is in effect
Essential Services Policy
Even on days when classes are canceled or the Springfield campus is declared closed due to inclement weather or other extraordinary situation, the university must provide minimal, adequate staffing in “essential services.” It is up to the division heads to make sure such services are provided.
Employees providing those services are required to report to work if so informed by their supervisors.
Units providing “essential services” will have a written plan for providing these services, including which employees and/or positions are needed, and will keep these plans on file with their division head.
